Muscle regains force long before repaired tissue regains its tolerance for repeated, rapid, unplanned loading.
Passing the strength test does not mean the tissue is ready
Nine months after a knee reconstruction, an athlete hits ninety percent of the other leg on every strength measure in the building, and gets cleared. Then they reinjure inside a year at a rate that has barely moved in two decades of improving surgery. Something in that sequence does not follow, and I do not think it is the surgery.
Force production is the fastest thing to come back because most of the early loss was never structural. Muscle shuts down after injury and after an operation partly through reflex inhibition, and when that inhibition lifts, strength returns quickly. Rebuilding a graft is a slower and stranger business. Transplanted tissue goes through a phase where its own cells die off and host cells repopulate it, and during that period the graft is weaker than it was on the day it was fixed in place. Only afterwards does the collagen reorganise along the lines of load, and that reorganisation is driven by the loading itself, over months.
So a strength test asks a question the tissue is not being examined on. Quadriceps torque at a fixed speed on a machine tells you about a muscle. It says nothing about whether a ligament can survive a rotational load applied in forty milliseconds while the athlete is looking at somebody else.
The second gap is neurological. After a knee injury, athletes change how they use vision to control the limb, relying more on watching what they are doing and less on the joint's own sense of position. That works in a gym where nothing unexpected happens. In a match, the eyes are busy with the ball and the opponent, and the limb is left with a degraded feedback system at the exact moment it needs the best one.
Here is my actual complaint. Clearance is usually a snapshot, taken rested, in a quiet room, on a good day. Reinjury happens tired, distracted, off balance, in the last twenty minutes.
Any test that does not include fatigue and surprise is testing a different athlete.
The ones who come back well are almost always the ones who spent longer in the boring middle, absorbing load, before anyone let them look impressive on a machine.
